Algunas pruebas diagnósticas, tratamientos, intervenciones y/o hospitalizaciones requieren prescripción médica y/o autorización previa de AXA/Agrupació, por ello, te detallamos varias formas para poder solicitarla.

Solicita y consulta tus autorizaciones y las de tus hijos menores de 18 años en MyAXA app y web de forma ágil y sencilla.
Puedes hacer seguimiento de tu autorización, adjuntar nueva documentación así como descargarte la autorización.

Para solicitar tu autorización debes tener a mano los datos de tu tarjeta, la prescripción de la prueba y/o tratamiento y el nombre del médico prescriptor y te facilitaremos el número de autorización

¿Qué información necesitas para solicitar tu autorización médica?

Debes solicitar tu autorización con menos de dos meses y más de 48 h antes de la realización de la prueba o intervención. Además, si es un ingreso por urgencias, deberás pedir la autorización durante las primeras 72 horas siguientes al ingreso. ¿Qué datos precisamos?

  • Población y provincia donde se realizará la prueba y/o intervención

Recuerda que AXA/Agrupació podrá solicitarte toda la información médica o clínica que considere necesaria para la valoración y tramitación de la prestación solicitada.

Si necesitas enviar algún tipo de informe médico, puedes adjuntarnos los siguientes modelos cumplimentados y firmados por el médico.

También, si lo prefieres, puedes firmar este consentimiento y permitir que AXA/Agrupació se ponga en contacto con tu médico para que nos facilite directamente la información del proceso por el que necesitas la autorización de la prueba médica y/o servicio médico asistencial.

Which tests, treatments, services or procedures require medical authorisation?

  • Aerosol therapy

    Ear, nose and throat: evoked potentials, electronystagmography, electrooculography, computerised vestibular tests.

    Neurophysiology: any functional study or test (e.g. electroencephalogram).

    Ophthalmology: fluorescein angiography. Special ophthalmological tests: visual evoked potentials.

    Radiology and diagnostic imaging: magnetic resonance imaging (MRI), computerised tomography (CT-scan), 3D full body X-ray.

    Rehabilitation: any treatment or physiotherapy session for the musculoskeletal system.

  • Any hospital admission.

    Any surgery or childbirth.

    Prostheses covered: stents (except aorta), coils, surgical meshes in hernia and incontinence surgery; mechanical and biological heart valves, vascular bypass prostheses, pacemakers, implantable pacemaker defibrillator; port-a-cath, chemotherapy infusion pump; monofocal IOL; osteosynthesis material and other internal prostheses in orthopaedic surgery; breast reconstruction following mastectomy (excluding the contralateral implant). Testicular prosthesis following testicular removal surgery due to cancer. Dura mater absorbable implants (exclusively in intracranial or spinal tumour cirrhosis). Only for reimbursement policies: CSF shunt valves in hydrocephalus, spinal implants and stabilisers, oesophageal and colon prostheses, IOL (multifocal) in cataract surgery and posterior chamber IOL (ICL).

    Allergy: provocation tests.

    Cardiac and vascular diagnostic tests: arteriography, catheterisation, heart ablation and cryoablation.

    Digestive system: diagnostic and therapeutic endoscopy (e.g. polypectomy and mucosectomy), hepatic elastography, oesophageal manometry and pH monitoring, lithotripsy (gallbladder and bile duct).

    Ear, nose and throat: laryngoscopy, evoked potentials, electronystagmography, electrooculography, computerised vestibular tests, laryngostroboscopy, dynamic posturography.

    Genetic testing, hormonal or immunological studies.

    Gynaecology and obstetrics: amniocentesis or amnioscopy, childbirth preparation, special gynaecological radiology (hysterosalpingography) and obstetric or gynaecological Doppler echocardiography.

    Nephrology: dialysis and haemodialysis, extracorporeal shockwave lithotripsy and laser intraurethral lithotripsy.

    Neurophysiology: any functional study or test (e.g. polysomnography, EEG).

    Nuclear medicine: any examination or treatment with radioisotopes. PET-CT 18FDG and PET Choline for monitoring and diagnosis in oncology. In reimbursement options, PET-CT for refractory epilepsies.

    Oncology: combined chemotherapy treatments, radiotherapy and the tests required for these treatments.

    Ophthalmology: fluorescein angiography, ophthalmic, orthoptic and pleoptic laser (covered only in children aged 6-10), visual evoked potentials.

    Pain management: nerve blocks and radiofrequency treatments.

    Radiology and diagnostic imaging: interventional radiology, vascular radiology, magnetic resonance imaging (MRI), computerised tomography (CT-scan), multidetector computed tomography (MDCT), 3D radiology of the spine.

    Rehabilitation: any treatment or physiotherapy session for the musculoskeletal system, vocal cords (phoniatrics/speech therapy), postpartum pelvic floor recovery and mild urinary incontinence. Shock waves. Cardiac rehabilitation in reimbursement options.

    Respiratory medicine: bronchoscopy, endobronchial ultrasound (EBUS), oxygen therapy, ventilotherapy, respiratory polygraphy.

    Urology: urodynamic and/or endoscopic assessment; GreenLight, Thulium and Holmium lasers in prostate surgery.

    Medical check-up.

    Psychological consultations or assessments.

  • Any hospital admission.

    Any surgery or childbirth.

    Prostheses: prostheses for heart valves, with the exception of those implanted via percutaneous catheterisation, pacemakers, internal orthopaedic prostheses, vascular bypasses, vascular stents and vascular coils. Bifocal or monofocal intraocular lenses, and breast and testicular prostheses.

    Allergy: provocation tests.

    Cardiovascular diagnostic tests.

    Digestive system: diagnostic and therapeutic endoscopy (e.g. polypectomy and mucosectomy), hepatic elastography, oesophageal manometry and pH monitoring, lithotripsy (gallbladder and bile duct).

    Ear, nose and throat: laryngoscopy, electronystagmography, electrooculography, computerised vestibular tests, laryngostroboscopy, dynamic posturography and laser treatment in surgery for tonsils, adenoids and Renke’s oedema.

    Genetic testing.

     Gynaecology and obstetrics: amniocentesis or amnioscopy, childbirth preparation course, gynaecological radiology and obstetric or gynaecological Doppler echocardiography.

    Nephrology: dialysis and haemodialysis, extracorporeal shockwave lithotripsy and laser intraurethral lithotripsy.

    Neurophysiology: any functional study or test (e.g. polysomnography, EMG,EEG) and visual and auditory evoked potentials.

    Nuclear medicine tests

    Oncology: combined chemotherapy treatments, radiotherapy and the tests required for these treatments.

    Ophthalmology: fluorescein angiography, YAG and argon laser ophthalmic surgery, orthoptics, tomography, HRT.

    Pain management.

    Radiology and diagnostic imaging: interventional radiology, vascular radiology, magnetic resonance imaging (MRI), computerised tomography (CT-scan), breast CT scan, multidetector computed tomography (MDCT), PET.

    Rehabilitation: any type of treatment.

    Respiratory medicine: bronchoscopy, endobronchial ultrasound (EBUS), oxygen therapy, ventilotherapy, respiratory polygraphy, continuous positive airway pressure (CPAP).

    Urology: urodynamic and/or endoscopic assessment and laser treatment.
